Population Health

 
Diamond has developed a population health model for behavioral health which includes utilization projections, processes for patient assessment, customized treatment protocols and outcome standards. The Diamond model has three primary goals, all of which fit the needs of a hospital, healthcare system or independent physician practice: 
 
  • Improve the overall health of a given clinical population
  • Improve the patient care experience
  • Reduce costs
 
The link between physical and mental health is clear and confirmed. Patients with a serious mental illness, particularly older adults, have multiple comorbid medical problems. Meanwhile, the volume and acuity of behavioral health patients in primary care offices and emergency departments (EDs) confirm the need for timely access to behavioral health resources.

MD Value Care - Richmond, VA
Developed and manage the behavioral health services for an ACO of approximately 14,000 Medicare patients in five primary care clinics

Embedded licensed provider with goals to:

  • increase access to behavioral healthcare, reduce length

  • of stay when hospitalization is warranted and minimize

  • inappropriate utilization of the ED

Alaska, Oregon and Washington - Design of a clinically integrated behavioral health service line for a ten hospital healthcare system with medical centers, critical access hospitals and clinics.
 
Arizona - Design of a clinically integrated behavioral health service for an urban health system client with multiple sites in the Phoenix metropolitan area; hospital and physician member ACO with no existing behavioral health services.
 
Massachusetts - Design of a clinically integrated behavioral health service for a regional hospital provider with a Medicare Shared Savings Program ACO.
 
Montana - Design of a clinically integrated behavioral health service line for an existing client within a regional faith-based health system; broad rural service area with an ACO and Patient-Centered Medical Homes.
 
Pennsylvania - Design of a clinically integrated behavioral health service for a network of four hospitals, addiction treatment and outpatient clinics in the Philadelphia area.
